ADULTS: PO 5 to 20 mg qd. Dosage should be individualized; some patients may be given starting dose of 2.5 mg/day.

Pregnancy: Category C. Lactation: Undetermined. Children: Safety and efficacy not established. Anaphylaxis: Deaths have occurred; aggressive therapy may be required.
